Abstract

Abstract Background and objectives: Acquired immunodeficiency syndrome (AIDS) is acquired by infection with human immunodeficiency virus. (HIV).Parasite caused infection is the most common problem in AIDS Patients. Sometimes unusual and opportunistic infections such as intestinal parasitic infections may cause serious gastric intestinal (GI) disorders, which can finally lead to death. The aim of this study is to find intestinal parasites in AIDS patients referred to Disease Consultation Center of Kermanshah province. Materials and methods: In this study, the stool samples were collected from 75 AIDS patients admitted to Kermanshah consultation disease center. Then, all samples were tested by direct and formalin ether methods. Results: As shown in results , 13 patients (17.4%) are infected with Entamoeba coli, two( 2.7%) with Entamoeba histiolyticaly,one(1.4%) with Giardia lumbelia ,six (8%) with Blastocystis hominis ,two( 2.7%) with Isospora belli ,one(1.4%) with Ascaris lumbericoides ,two( 2.7%) with Endolimax nana . Conclusion: This study indicated that the frequency of pathogenic and non-pathogenic parasites was low in HIV+ patients. This may be related to anti-parasitic drugs used in all HIV+ patients admitted to Kermanshsh nsultation Disease center. Abstract

Abstract Background and objectives: Vibrio Cholera is one of the causative agents that can easily lead to death if the appropriate therapeutic measures are not taken in time. The purpose of this study is to identify the antibiotic sensitivity of the isolated V. choleras from the patients during the outbreak of Cholera in Golestan province, 2005. Materials and Methods: The Subjects were 97 positive cultured samples sent by different laboratories of Golestan province health centers .First, these samples were confirmed with microbiological and biochemical tests. Then, their antibiotic sensitivity was examined by Kirby –Bauer method (12 routine drugs were used).To obtain better results, the tests carried out in parallel by two expert technologists. Results: The participants, aged 29.8 +- 16 years, are male (52.6%) and female (47.1%). The highest antibiotic susceptibility pattern is related to Cephotaxime (91.6%), Ceftizoxime (87.8%), Oxcy Tetracycline (76.1%) while the most antibiotic resistance is referred to Nalidixic Acid (84.5%), Fourazolidin (50.7%) and Erythromycin (50.7%). During outbreak, Antibiotic resistance was not increased. Both technologists reports regarding Ampicillin, Tetracycline, Nalidixic Acid (kappa of 94%, 89%, 82%, p value<0.05) have the most agreement while it is not true about Erythromycin and Fourazolidin disks. (Kappa 67% and 62%, p value<0.05) Conclusion: Because of the resistance of Vibrio cholera to different antibiotic agent, it is important to determine their pattern to control and prevent the spread of drug resistance species. Abstract

Abstract Background and objectives: Ureaplasma urealyticum is one of the most important causes of Nongonococcal urethritis (NGU) and non-specific urethritis (NSU) in men. Also Mycoplasma hominis has a role in causing NGU and NSU. The aim of this study was the detection of Mycoplasma hominis & Ureaplasma urealyticum in men suffering from NGU and NSU by prostatic massage method and first- void Urine samples. Material and Methods: Urethral secretion samples, taken after prostatic massage and first-void urine, of 75 patients referred to department of bacteriology of Tehran University of Medical Sciences were examined for ureaplasma and mycoplasma by culture method in 2004-2005 Results: In prostatic samples, 19 and 11 samples are positive for ureaplasma and Mycoplasma, retrospectively. In first-void urine samples, 17 and 9 samples are positive for ureaplasma and Mycoplasma, retrospectively. Conclusion: The results show that if it is impossible to obtain urethral sample via prostatic massage , we can use first- void urine instead. Abstract

Abstract Bachground and objectives: Improvement of the effectiveness of services is one of the most important strategies in many clinical and research laboratory centers. The increased client satisfaction (researchers or patients), resource development innovation efficacy, continuity of services and income are also the important strategies of these centers. For achieving these strategies, we combined the qualitative and the quantitative approaches to evaluate the effective model based on frameworks of the Balance Scorecard (BSC) and the Laboratory Quality Management Systems (LQMS). Material and Methods: This paper describes the basic steps required for designing and developing of a model for performance measurement in laboratory centers. For model validation, the relationship between the criteria, the strategies and the strategic objectives was evaluated using both the AHP method and the evaluation by the experts. Results: Implementation of the above mentioned model, conducted in one of the largest Laboratory centers, has resulted in a remarkable enhancement in the efficiency (22%), profit (33%), and level of quality of service (27%) and in the clients' satisfaction (4%). Conclusion: This research model is a valuable tool for effective evaluation and continuous improvement of the clinical and research laboratory centers. Applying this model leads to increased resource productivity as well as the clients and community satisfaction from the laboratories services. Abstract

Abstract Background and objectives: Identification and monitoring of multidrug-resistant Mycobacterium tuberculosis strains (MDR) is highlighted by the high risk of their spreading in different areas. Prevalence of these strains was evaluated in Golestan province in northeast of Iran. Material and Methods: Drug susceptibility testing to Isoniazid and rifampin was carried out for 148 clinical samples that had grown in Mycobacteria growth indicator tube (MGIT) system, according to the manufacturer's instructions (Becton-Dickinson, USA). The association of drug resistance frequency with demographic characteristics and growth time were investigated. The appropriate statistical tests, X2 and student T- test were performed for comparison of these variants. A p value>0.05 was considered significant in all cases. Results: The turnaround time required for growth of Mycobacterium tuberculosis in MGIT system was between 2 to 55 days (mean 16.3±10.4 days). Of all samples studied, 17.6% and 3.4% were resistant to Isoniazid and rifampin, respectively, and 3.4% (5 samples) were MDR (CI 95% 1-6%). The turnaround time required for determining MDR cases was 9.6 days. No statistically significant association was found between the resistance to the drugs and none of the factors including sex, age, type of clinical sample, and positivity of the smear. Conclusion: The prevalence of MDR in the studied region was determined to be 3.4% which is similar to the country-wide evaluations. The turnaround time for Mycobacterium growth and anti drug susceptibility result can be shortened by MGIT method. Abstract

Abstract Background and objectives: Diabetes is a chronic and progressive disease resulting in disability and premature death. The Etiology of diabetes mellitus is complex, but factors such as genetic, immunological and environmental are involved .The aim of this study was to investigate the association between diabetes and blood type. Material and Methods: In this cross - sectional study blood group of 500 patients with diabetes type 2 was determined and compared with the distribution of blood groups in 11,461 healthy people in Ahwaz. Statistical analysis was performed by independent T test, using SPSS Software (version 17) Results: the Patients’ age is between 35 and 80 years. Their Blood groups are A (106 20.3 %) B (144 27.5%) AB (97 18.5%) and O (176 32.7%). One hundred seventy-nine (34.2%) cases are Rh+ and the rest (344 65.8 %) Rh- . Conclusion: Regarding to the distribution of blood group AB and Rh- ,which respectively are 7.3% and 8.4%, the chance of developing diabetes in theses people is higher than that in other people (P<0.05). Hence, they should follow a healthy lifestyle from early years of life to reduce the risk of diabetes. Key words: Diabetes type 2, blood group, risk factors

Abstract

Abstract Background and objectives: Recurrent miscarriage (RM) is one of the most common reproductive disorders, generally considered to be the loss of three or more pregnancies before viability. One of the causes of this disorder is the immunological factors such as autoantibodies associated with anti-phospholipid syndrome. Material and Methods: this case-control study was conducted on 80 pregnant women divided into two equal groups of healthy and RM women in Gorgan. The subjects were asked to fill out a questionnaire and examined by a gynecologist to distinguish their condition, healthy or RM. The blood samples were kept at -20c and assessed by both ELISA 96 and automated ELISA processor with single dose ready-to-use test to measure β2GP1 autoantibodies of IgM and IgG. Results: the results indicate that % 7.5 of RM patients are seropositive for Anti-β2GP1 IgG, and % 5 for Anti-β2GP1 IgM , but in healthy pregnants , % 7.5 are seropositive for Anti-β2GP IgG , and %7.5 for Anti-β2GP1 IgM. Family history of abortion in RM group (33.5%) is significantly higher than healthy one (17.5%). Conclusion: based on the results, the difference between the level of Anti-β2GP1 IgM and Anti-β2GP IgG in case and control groups is not significant, but the production of this autoantibody is associated with pregnancy. Abstract

Abstract: Background and Objective: A wide variety of opportunistic pathogens has been detected in hospital surfaces. Among these , Pseudomonas species are one of the leading causes of nosocomial infections, frequently found in hospital environments. The purpose of this study was identification of antimicrobial susceptibility of Pseudomonas spp. isolated from different Section of ShahidRajaeii hospital, Tonekabon. Material and Methods: the samples (460) from different sections of Shahid Rajaeii hospital, Tonekabon were collected between December 2010 and June 2011. The identification of the strains was performed by using biochemical tests and API20NE (Biomerieux) ,and antimicrobial susceptibility of the isolates against different antibiotics was determined by disc diffusion test. Results: of 460, 61(%13/26) strains of Pseudomonas are isolated from all the sources. The highest rate of Pseudomonas spp. is recorded in Surgery and ICU, while the lowest in Dialysis ward. Of 61 strains of Pseudomonas, 52 (85/25%) are belonged to Pseudomonas aeruginosa, six (9/83%) to Pseudomonas stutzeri, two (3/28%) to Pseudomonas putida and one (1/64%) to Pseudomonas fluorescens. Conclusion: the environments of the hospital can be the vehicles of Pseudomonas spp. therefore, both the patients and personnel should have extra attention to their personal hygiene to avoid Pseudomonas infection. Abstract

Abstract Background and Objective: Resistance to antiretroviral agents is a significant concern in clinical management of HIV-infected individuals. Resistance is the result of mutations that develops in the viral protein targeted by antiretroviral agents. Material and Methods: In this cross-sectional study, the blood samples of 40 HIV-positive patients were collected. Twenty of them were drug-naïve and the rest were under treatment for at least one year by antiretroviral agents. Virus genome was extracted from patient's plasma with high-pure-viral-nucleic-acid kit. Then, by means of reverse-transcriptase and specific primers of protease genes were amplified and sequenced. Sequences of genes, drug- antiretroviral- resistant mutations and subtypes were determined using Stanford University’s HIV-drug-resistance databases. Results: Drug-naive patients show 15% resistance to nucleoside-reverse-transcriptase inhibitor (NRTI) and 20% resistance to non-nucleoside-reverse-transcriptase inhibitor (NNRTI). Anti-protease resistance is not observed in any patients. In under treatment patients, drug resistance to NNRTI (25%) is more than drug resistance to NRTI (20%) and the rate of drug resistance to protease inhibitor is 5%. Conclusion: Our findings show a high prevalence of drug-resistant mutations in Iranian-drug-naïve-HIV-infected patients. Abstract

Abstract Background and Objective: One of the diabetes complications is the tissue damage caused by the imbalance of oxidants and antioxidants (oxidative stress). The aim of the present study was to evaluate the activity of two antioxidant enzymes -superoxide dismutase and catalase- in the serum of streptozotocin-induced diabetic rats. Material and Methods: This investigation was conducted on adult male rats assigned to diabetic and control groups. Diabetes was induced by a single intraperitoneal injection of streptozotocin. Seven weeks after diabetes induction, glucose concentration, superoxide dismutase and catalase activities of the serum were assessed. Results: Glucose concentration of streptozotocin-injected animals was significantly higher than that of control group (P<0.001). The level of Serum superoxide dismutase and catalase activities in diabetes group were significantly higher than those in control group (P<0.01). There was a positive significant correlation between glucose concentration and superoxide dismutase and catalase activities (P<0.001). Conclusion: The high activity of antioxidant enzymes in diabetic rats is probably due to compensation responses to oxidative stress produced by high concentration of free radicals. Abstract

Abstract Background and Objective: This study was aimed to determine the extent of bacterial contamination and drug resistance patterns of isolates colonized in colonoscope and endoscope and in relevant personnel. Material and Methods: A total of 107 samples were obtained from staff of endoscopy and colonoscopy units (SEU and SCU) and gastroenterological imaging equipment. For isolation and identification of the bacteria, swab culture method and biochemical identification test were used, respectively. Antimicrobial resistance profiles, multi-drug resistance (MDR) patterns and phenetic relatedness of these isolates were also analyzed according to standard methods. Results: Most frequent pathogenic bacteria among the SEU and gastroenterological imaging related equipments were included S. aureus (20.8 % and 0 %) Enterococcus spp. (0 % and 5.4%) Pseudomonas spp. (0% and 13.5 %), and Clostridium difficile (0% and 12.5%). Analysis of resistance phenotypes showed a high frequency of MDR phenotypes among the SEU (82.1%), and also in endoscopes, colonoscopes, and other equipments (20%, 50% and 100%, respectively). Phylotyping of S. epidermidis isolates showed the role of staff in transmission of resistance strains to medical equipments and also circulation of strains with identical resistance phenotype among the studied samples. Conclusion: High frequency of pathogenic bacteria in colonoscopes, endoscopes and in the staff of endoscopy & colonoscopy units, and also contamination of these instruments with MDR pathogens emphasize the need for proper disinfection of endoscopes and colonoscopes and also instruction of staff in these units. Abstract

Abstract Background and Objective: Mycoplasma hominis and Ureaplasma urealyticum are recognized as the main pathogens of urogenital tract. Mycoplasma genitalium can cause several reproductive tract inflammatory syndromes in women. Material and Methods: This study was conducted on 100 women with the history of abortion. The samples were obtained from cervical secretions and remaining products of curettage to diagnose Genital Mycoplasma. All samples were inoculated into PPLO Broth medium and incubated at 37ºC for 3 days. Results: Of 21 Mycoplasma isolates, Mycoplasma hominis was isolated from both cervical secretions and remaining products of curettage (4.26%) Ureaplasma urealyticum was isolated from products of curettage (12.8%), cervical secretions (8.5%) and from both samples (6.38%). Conclusion: Based on the results, a high percentage of women with the history of abortion were infected with these bacteria. Thus, early diagnosis and treatment of these pathogenic organisms are necessary. Key words: Mycoplasma Hominis Ureaplasma Urealyticum Abortion

Abstract

Abstract Background and Objective: Transmission of pathogens by cosmetics is one of the major health complications. Direct contact with contaminated non-standard cosmetics can have irreparable side effects for the consumers. Thus, the evaluation of microbial contamination in cosmetic products is important. The aim of this study was to assess the microbiological contamination of one of frequently used cream. Material and Methods: In the present study, 135 samples of a special moisturizing cream were randomly selected from pharmacies in Tehran. The microbial contamination assessment, sampling and culturing method were based on the protocol (No.3978) of Iranian Institute of Standard and Industrial Research. Results: sixty-two (46%) out of 135 samples were contaminated. The highest and lowest contaminations observed were Pseudomonas aeruginosa and Bacillus, respectively. Conclusion: Due to the high contamination rate of cosmetic creams, we recommend extremely monitoring and controlling these products by health centers. Abstract

Abstract Background and Objective: Culture, microscopic method is a gold standard method for identification of Lishmania parasite. The use of Molecular methods such as RT- PCR compared to microscopic methods has a higher sensitivity and specificity however, it is not widely used due to its expensive equipment and the time requested. The use of nucleic acid sequence based amplification (NASBA) method is highly valuable for diagnosis of live parasite because there is no need for to use Thermo cycler. We aimed to assess sensitivity and specificity of NASBA for molecular detection of cutaneous Leishmaniasis. Material and Methods: First, the RNA was extracted from 28 skin biopsies suspected cutaneous Leishmaniasis. Then, by means of specific primers designed for 18srRNA region, this region was amplified using NASBA isothemal amplification. To increase the sensitivity, the product was electroforesed in TBE (IX) buffer, using Syber Gold Flourecent probes. Using specific primers, RT- PCR was conducted on the samples too. Result: For diagnosis of Leishmania parasites, NASBA and RT-PCR had the sensitivity of 81% and 51%, respectively, and specificity of 100%. Conclusion: NASBA isothermal method with high sensitivity and specificity can be applied for identification of cutaneous leishmaniasis. Keywords: Cutaneous Leishmanisis, NASBA, 18S rRNA

Abstract

Abstract Background and Objective: Because of increased resistance to antibiotics, side effects of chemical drugs and importance of medicinal plants, we aimed to assess the antibacterial effects of methanolic extract of the Polygonumbistorta plant on the E. coli (ATCC 15224), Ps. aeruginosa (ATCC 25619), B. subtilis (ATCC 6633) and Stap. Aureus (ATCC 25923). Material and Methods: After preparing the extract, its antibacterial effect was assessed via gel diffusion method, using disk / well diffusion methods to determine MIC and MBC Results: MIC of methanolic extract was 78 µg/ml for E. coli, 63×103 µg/ml for Pseudomonas aeruginosa, 39 µg/ml for Bacillus subtilis and 31×102 µg/ml for Staphylococcus aureus Conclusion: In spite of resistance of gram-negative bacteria to chemical agents, polygonum bistorta methanolic extract could inhibit the growth of E.coli and P. aeruginosa. Abstract

Abstract Background and Objective: Bacterial colonization in upper respiratory airways is one of the major risk factors for the development of the ventilator–associated pneumonia (VAP), which is the most common and serious hospital-acquired infection in intensive care unit (ICU). The aim of this study was to determine the frequency of oropharyngeal microorganisms of patients with tracheal tube hospitalized in ICU. Material and Methods: Of 39 patients hospitalized in ICU of panje Azar Hospital, the oropharyngeal cultures were taken after admission. The samples were evaluated for growth of Staphylococcus aureus, Pneumococcus, Enterococcus, Pseudomonas, and E-coli. Results: The mean age of the patients (21 men, 18 women) was 43.64±15.01. The culture was positive in 28.2% and the most common isolate was Pseudomonas aeruginosa (10.3%). Conclusion: Pseudomonas, which is the main pathogen for ventilator- associated pneumonia, may be a potential threat for the patients hospitalized in intensive care units. Abstract

Background and Objective: Cytomegalovirus (CMV), one of the most common opportunistic pathogens in patients infected with human immunodeficiency virus (HIV), can cause the diseases such as encephalitis, pneumonia, and chorioretinitis. This study aimed at molecular studying of CMV infection in individuals infected with the human immunodeficiency virus. Material and Methods: In this study, 50 venous blood samples from HIV-infected individuals were taken. Patients were divided into two categories: patients under treatment with and without antiretroviral drugs. Plasma were separated from blood samples and examined for the presence of cytomegalovirus genome by PCR. Material and Methods: this study was conducted on 50 blood samples from HIV-infected individuals, and plasma was separated and examined for the presence of cytomegalovirus genome by PCR. Patients were divided into two group of under treatment with and without antiretroviral drugs. Results: Of 50, 28 (% 56) were men and 22 (% 44) were women. CMV genome was identified in 8 samples (16%), and the molecular prevalence of CMV infection was 21.4% (n= 6) in males and 9.1% (n = 2) in females. Conclusion: Given the frequency of Cytomegalovirus Active Infection in HIV-infected individuals under antiretroviral therapy, we should be careful about the treatment of Cytomegalovirus Active Infection. Abstract

Abstract Background and Objective: Klebsiella pneumoniae is one of the agents causing nosocomial infection therefore, we decided to report the prevalence of Klebsiella pneumoniae caused infection. Material and methods: The frequency of Klebsiella in culture media samples of Panje Azar hospital was studied in 2011-2012. After determination of the species with biochemical methods and determination of resistance to third generation cephalosporins, the existence of responsible genes for this resistance was investigated using specific primers. The PCR product for CTX-M gene was sequenced. Results: During the study, 70 isolates of Klebsiella were isolated in that 51 (72.8%) related to three months of November, December and January. Except for the one related to November, other ESBL cases belonged to these three months. Based on molecular investigation of ESBL genes, these isolates at least were in 3 types and had a high frequency in Internal, female and Emergency wards. Conclusion: The present report implied a sudden prevalence of Klebsiella pneumoniae that detected and controlled by a correct monitoring. Keyword: Klebsiella Pneumoniae, ESBL, CTX-M

Abstract

Abstract

       Background and Objective: Antibiotics are the chemical compounds, which are produced by microorganisms, belong to a larger group of antimicrobial compounds that are used for treatment of infections caused by microorganisms.

        Material and Methods:  the isolates of four plant species, Astragalus obtusifolius, Prosopis juliflora, Xanthium strumarium and Hippocrepis unisiliqousa were obtained using Trypticase Soy Agar. First, the purified isolates were screened from the viewpoint of their ability in producing antimicrobial metabolites against three typical microorganisms Escherichia coli, Candida albicans and Saccharomyces cerevisiae. Then, the microbial products were extracted using organic solvent ethyl acetate and their minimum suppression concentration was determined against three strains Candida albicans ATCC 10231، Staphylococcus aureus ATCC 25923 و Escherichia coli ATCC 25922.

         Results: After 4-7 days of incubation, 104 bacterial samples were isolated from isolates four plants. Out of this number, 26 isolates were antimicrobial producing metabolites by making inhibition zones against three typical microorganisms, after initial screening for production of antimicrobial compounds using agar diffusion method. Minimum inhibitory concentration of ethyl acetate extracts from target microbes were between 125-1000 µg/ml.

        Conclusion: The results showed that the bacterial extracts of phylosphere produce some compounds with antimicrobial properties.
